Summary
We are seeking a dedicated and passionate individual to join our team as a Community Work Transition Program Specialist. This full-time position. The ideal candidate will have a strong understanding of community resources, vocational training, and job placement strategies, and will work closely with participants to help them achieve their employment goals.

Responsibilities
- Develop and implement individualized transition plans for participants, focusing on their unique strengths, interests, and career aspirations.
- Collaborate with local businesses, educational institutions, and community organizations to create job opportunities and internships for participants.
- Provide job readiness training, including resume writing, interview preparation, and workplace etiquette.
- Conduct assessments to identify participants' skills, interests, and barriers to employment.
- Facilitate workshops and training sessions that enhance participants' employability skills.
- Monitor and evaluate participants' progress throughout the program, adjusting plans as necessary to ensure success.
- Advocate for participants in various settings, including schools, workplaces, and community organizations.
- Maintain accurate records of participant progress, job placements, and program outcomes.
- Stay informed about best practices in vocational rehabilitation and community resources to provide the most effective support.
- Prepare reports and presentations for stakeholders, highlighting program successes and areas for improvement.

Requirements
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations.
- Proven ability to develop and maintain relationships with employers, community organizations, and educational institutions.
- Strong organizational and time management skills, with the ability to manage multiple priorities effectively.
- Proficient in Microsoft Office Suite and experience with data management systems.
- Knowledge of local labor market trends and resources available for job seekers.
- Ability to work independently and as part of a team, demonstrating initiative and creativity in problem-solving.
- Valid driver’s license and reliable transportation for community outreach and job site visits.
